Report a Suspected Case

The form below shall be used to notify QatarEnergy of any person who is working within the Energy Sector and is determined to be a Suspected Case. A person is considered a Suspected Case if the following criteria apply:
  • ​A body temperature of > 37.8 degrees Celsius; AND / OR
  • Other symptoms of flu (e.g. cough); AND / OR 
  • Has been in Close Contact with a CONFIRMED case within the past 14 days.

‘Close Contact’ means more than 15 continuous minutes within 1.5 metres of the person.

INSTRUCTIONS
1.  All fields MUST be filled.
2.  All fields must be filled in EXACTLY the same format as shown.
3.  Failure to complete the form correctly may result in the decline of your request.
4.  Appointment time at the Clinic will be sent by SMS to the individual's cellphone AND by email to the registered email.
5.  All people arriving for testing MUST present their QID or Passport if they don’t have a QID.
